When would I use a pen-grip luxator?
A pen grip can be used with any instrument when you need fine control and reduced force situations where
precision matters more than power. Specifically:
Working on small or narrow-rooted teeth (incisors, small premolars in cats and small dogs), where
excessive force risks fracturing the root or the thin surrounding alveolar bone.
Initial ligament sectioning, when you’re working the blade circumferentially around the tooth and need
controlled, shallow insertions rather than deep leverage.
Working near sensitive anatomical structures (e.g., near the mandibular canal, nasal cavity, or thin
cortical bone in cats), where a slip or excess force carries higher risk.
Fine tactile feedback – the pen-grip (holding the instrument like a pen, fingers close to the working
end) gives you better proprioceptive feedback on how the blade is engaging the ligament, useful when
feeling your way through the fibrous attachment rather than forcing through it. Alternatively, holding it
further back on the handle also narrows your working profile, helping you access tight spaces.
The trade-off is that a pen-grip sacrifices power for control – you wouldn’t use it when you need real
leverage (that’s when you switch to a palm-grip, and eventually to an elevator). So pen-grip is the right call
early in the luxation process, on delicate teeth, or whenever the priority is precision over force.

What is the difference between a luxator
and an elevator?
A luxator has a sharper, thinner blade that
severs the periodontal ligament with a gentle
pushing motion. An elevator uses controlled
leverage and rotation to release the ligament
fibres and loosen the tooth. Both are used
in veterinary tooth extraction and are often
used together for optimal results.
Note: Do not use luxators as elevators, and
vice versa.
